The Polk County Health Department is now taking appointments for Pre-K and kindergarten round up.

All students will need an updated immunization certificate and a hearing, vision and dental screening. Even if your child had the hearing, vision and dental screening last year, they will need another one to start kindergarten.

An immunization certification and screening can be obtained at the Polk County Health Department.

April 9-13 have been set aside for these screenings. Please call 770-749-2270 for an appointment.
